Enrolling a New Student at LIFE
These are the steps to be followed to enroll a new student at LIFE! Lutheran
School:
1. Set up an appointment with the principal before meeting with the teacher(s).
2. Complete the New Student Enrollment
Form, Financial Agreement Form and Volunteer Commitment Form. Complete
and return the enrollment forms promptly, since the date received may also
become a factor during Open Enrollment. The registration fee (and book
fee, if applicable) must be returned with the enrollment forms. Be
sure to fill out all of the parent information. Click on the form to download
a PDF version of the form.
3. If the student has not previously been enrolled in a school in the state
of Oregon, an
Immunization Certificate (available from the school office) must be completed
and returned with the application form. Otherwise, we can request a transfer
of records from the previous school.
4. After you have returned your registration fee(s) and forms, you will
receive a letter of
confirmation if the student is accepted into enrollment at LIFE! Lutheran
School. You will
receive a tuition coupon booklet prior to your first tuition payment on
July 1st for Kindergarten through 8th grades (12 months), or prior to September
1st for Preschool (9 months).
5. Families requesting tuition assistance should pick up the Tuition Assistance
Assessment
Form from the school office after enrolling and follow all of the directions
on the form.
If you have any questions regarding enrollment at LIFE! Lutheran School,
please contact the
school office and we will be happy to answer your questions.
